Students age 18 and older may verify their own attendance. A parent or guardian must give them permission by submitting this form.
What to know
Fill out and submit this form if you are:
- 18-years-old or older, and
- Want to be able to verify your own attendance
You will need:
- Written authorization from your parents or guardians
- Approval from your associate principal
