Some students receive online mental health care during school hours. We need a signed authorization form so we can support them.
What to know
- The form must be signed by:
- Students over age 16 or older.
- Parent or guardians of students who are under age 16.
- The information you provide will help us arrange appropriate space and resources for the online appointment.
